Although first published in 1980, this book is still considered by many to be a seminal text in the field—the first text to offer a methodology for helping patients develop more adaptive responses to life's essential dilemmas. The book is organized around what Jalom identifies as four existential, and what he calls, supreme concerns – death, freedom, isolation, and meaninglessness, how they manifest in personality and psychopathology, and how our knowledge of them can aid in healing. These four questions are the content and process of the dialogue between therapist and patient. The book is written clearly and without jargon to be accessible to the lay reader, with many examples from Jalom's practice.

Yalom's formal definition of existential psychotherapy is that it is a dynamic approach to therapy focused on the concerns embedded in the very existence of the individual. Unlike other psychotherapy directions that focus on behavior, existential psychotherapy starts from the idea that all people are faced with fundamental existential dilemmas, how to find the meaning of life in a world that does not offer it to us, how to relate to our own mortality, how to take responsibility for our own choices and mistakes.

We all long for eternity, grounding, community and a role model; and we must all face inevitable death, helplessness, isolation and meaninglessness. Existentialist therapy is based on a model of psychopathology according to which anxiety and its consequences are the answer to four supreme concerns, and in practice this therapy means helping clients to recognize and accept uncertainty and find meaning and authenticity in their lives.
